WellSky Social Care Coordination's new "private resource sites" feature allows community-based organizations that offer sensitive programs and services (e.g., substance use, mental health, services for people living with HIV/AIDS, etc.) to securely participate in our accountable network.

This new feature ensures that referrals made to those sensitive programs are only seen by the organizations issuing the referral and the organizations receiving the referral.

Community-based organizations that are configured to receive "private referrals" will be prompted to capture a one-time client consent before updating the referral. See below for more details on this exciting new capability of WellSky Social Care Coordination.

Private In-Network Resource Sites

In-Network resource sites will have a privacy status of either Private or Not Private. A resource site that is Not Private means that anyone with access to the patient profile can see any and all referrals made to the resource site that is Not Private. If your patient profile is shared with another organization, that organization will be able to see referrals made to Not Private resource sites, for that patient.

The patient profile is shared with another organization when an In-Network referral is sent to another organization.

Private Resource Sites

When a resource site is marked as Private, referral visibility is limited. The referral made to the Private resource site can only been seen between the organization that sent the referral, and the organization that received the referral.

You will notice some In-Network resource sites will have an orange label marked Private. This is to alert you that the resource site is private and any referrals sent to it can only be seen between the sending organization and the receiving organization. This resource site that is Private may offer sensitive programs and services (e.g., substance use, mental health, services for people living with HIV/AIDS, etc.).

Logged Referrals

All Logged Referrals, or Out of Network referrals (learn more about what a logged referral is here.) will not be shared when the patient's profile is shared with another organization. All Out of Network referrals stay within the organization they were created on.

Private Referrals

In-Network referrals sent to a Private Resource site will be marked as Private. When a referral is marked as Private that referral can only be seen between the organization that sent the referral, and the organization that received the referral.

In addition to strict referral visibility, patient consent must be collected in order to post certain updates on the referral. Consent only needs to be collected one time - at the time the first update is made.

On the top of the referral page you will see a banner where your patient's consent must be captured in order to post your first update on the referral:

Referral Update Consent Language

Additional Client Consent Required

The initial referral to your organization is designated as private within the WellSky Social Care Coordination network due to its sensitive nature. Updates will be shared back with the referring entity, which includes the status of the referral (e.g. in-progress, completed, canceled), actions logged (e.g., client contact successful, client waitlisted, service provided), and specific services provided (14 weeks of meals, one 30 minute ride, 2 months of emergency housing).

โ˜‘ By checking this box, you are confirming that (1) you have reviewed this authorization request with your client; (2) you have reviewed this update with your client and your client understands that Wellsky will share all updates to this referral with the referring entity, ABC, and (3) your client authorizes sharing this information.

The following requires consent to be collected before the first update can be made:

The following actions do not require the confirmation of consent. You can make these updates without confirming consent from the patient:

Referral Privacy Status

You can easily see the privacy status of the referral in either the referrals section of the patient profile page, or in your Referrals tab, on the top navigation bar. A column titled Privacy Status will show as the last column.

Referrals marked as Private in the column are private referrals. Referrals without a privacy status (blank) are Not Private referrals.

Please note that all Out of Network referrals stay within the organization that created them - they are not shared in the patients profile with another organization. Learn more here.

Updated Privacy Status

If an In-Network site changes their privacy status, all past referrals will be affected:

When an In-Network site is changed to "Private"

  • All past referrals are made Private

  • Referrals moving forward will be private

When an In-Network site is changed to "Not Private" (and it was previously "Private)

  • All past referrals remain Private

  • Referrals from that point on will be not private (open visibility)
