The Healthify De-Identified Service Detail Extract enables customers with referrals to receive automatic exports of service detail information, excluding identifiable patient information. Customers may receive this file on a selected time interval (typically monthly or weekly) delivered as a zipped CSV file via either email or SFTP upload.

Each row represents a single service detail logged within Healthify during the report's date range, and the file will be filtered by the Network or Company, as well as the defined date range (i.e. past month, week, day, etc.). The rows will be sorted based on the Service Detail Created Time, Referral ID, and Associated Service Offering Name — so service details that share the same referral and service offering are grouped together.

Data Definitions

Fields are listed in the order they appear in the De-Identified Service Detail Extract.

Service Detail UUID: The secure unique identifier of the service detail. This is an alphanumeric ID which represents a single action logged for a single service offering within a referral. If the same action was logged for multiple service offerings at the same time, then multiple rows will be generated, each with a different UUID.

Service Detail Created Date: The date when the service detail was logged in the company or network's time zone. Formatted YYYY-MM-DD.

Service Detail Encounter ID: The unique identifier of the service detail encounter. An encounter is an action logged for one or more service offerings at a time.

Logging User ID: The unique identifier of the user who logged the service detail.

Logging User First Name: The current first name of the user who logged the service detail. (Note: If the user's name has changed at any point, this value will represent the most current name as of the time that the report was pulled.)

Logging User Last Name: The current last name of the user who logged the service detail. (Note: If the user's name has changed at any point, this value will represent the most current name as of the time that the report was pulled.)

Logging User Email: The current email address of the user who logged the service detail.

Logging Team ID: The unique identifier of the team of the user who logged the service detail. A team is a group of users which belongs to a company. (Note: If the referring user has changed teams after logging the service detail, this value will still represent the user's team when the service detail was logged, not the user's current team.)

Logging Team Name: The current name of the team of the user who logged the service detail. A team is a group of users which belongs to a company. (Note: If the team's name has changed at any point, this value will represent the most current name as of the time that the report was pulled.)

Logging Company ID: The unique identifier of the company of the user who logged the service detail. (Note: If the referring user has changed companies after logging the service detail, this value will still represent the user's company when the service detail was logged, not the user's current company.)

Logging Company Name: The current name of the company of the user who logged the service detail. (Note: If the company's name has changed at any point, this value will represent the most current name as of the time that the report was pulled.)

Referral ID: The unique identifier of the referral. This is a numeric ID that is also visible in the Healthify application.

Referral Type: Whether the referral was an In-Network - Preferred, In-Network, or Out-of-Network referral. In-Network - Preferred referrals are sent to services that participate in the specified network as In-Network Preferred Partners. In-Network referrals are sent to services that participate in the specified network as In-Network Partners. Out-of Networks referrals are logged within the Healthify platform, and these services do not participate in the specified network.

Referring User ID: The unique identifier of the user who created the referral.

Referring User First Name: The current first name of the user who created the referral. (Note: If the user's name has changed at any point, this value will represent the most current name as of the time that the report was pulled.)

Referring User Last Name: The current last name of the user who created the referral. (Note: If the user's name has changed at any point, this value will represent the most current name as of the time that the report was pulled.)

Referring User Email: The current email address of the user who created the referral.

Referring Team ID: The unique identifier of the team of the user who created the referral at the time of referral creation. A team is a group of users which belongs to a company. (Note: If the referring user has changed teams after creating the referral, this value will still represent the user's team when the referral was created, not the user's current team.)

Referring Team Name: The current name of the team of the user who created the referral at the time of referral creation. A team is a group of users which belongs to a company. (Note: If the team's name has changed at any point, this value will represent the most current name as of the time that the report was pulled.)

Referring Company ID: The unique identifier of the company of the user who created the referral at the time of referral creation. (Note: If the referring user has changed companies after creating the referral, this value will still represent the user's company when the referral was created, not the user's current company.)

Referring Company Name: The current name of the company of the user who created the referral at the time of referral creation. (Note: If the company's name has changed at any point, this value will represent the most current name as of the time that the report was pulled.)

Referring Network ID: The unique identifier of the network of the user who created the referral at the time of referral creation.

Referring Network Name: The current name of the network of the user who created the referral at the time of referral creation. (Note: If the network’s name has changed at any point, this value will represent the most current name as of the time that the report was pulled.)

Receiving Service ID: The unique identifier of the service to which the referral was made. A "service" is a specific social service or program provided by a community organization at one or more locations. Referrals are made to a single service at a single location.

Receiving Service Name: The current name of the service to which the referral was made. A "service" is a specific social service or program provided by a community organization at one or more locations. (Note: If the service's name has changed at any point, this value will represent the current name as of the time that the report was pulled.)

Receiving Service Location Zip Code: The zip code of the location’s address to which the referral was made.

Receiving Organization ID: The unique identifier of the organization which currently owns the service and location to which the referral was made. An "organization" is an entity which provides community or social services and may optionally own services and locations.

Receiving Organization Name: The current name of the organization which currently owns the service and location to which the referral was made. An "organization" is an entity which provides community or social services and may optionally own services and locations.

Receiving Company ID: The current Company ID of the service and location that the referral was sent to. Only present for In-Network and In-Network - Preferred referrals.

Receiving Company Name: The current Company Name of the service and location that the referral was sent to. Only present for In-Network and In-Network - Preferred referrals.

Service Detail Action: The action that was logged. Values are “Client Contact - Successful”, “Client Contact - Unsuccessful”, “Client Waitlisted”, “Client Enrolled in Service”, “Service Provided” and “Service Not Provided”.

Service Detail Action Date: The date that the action occurred, as reported by the user. (Note: this is self-reported information, and it may be a different date as the Service Detail Created Date.)

Associated Service Offering Name: The service offering for which the user is logging an action. Values come from Healthify's Service Offerings taxonomy.

Associated Service Offering Category Name: The social determinants of health (SDoH) domain category(ies) of the user-selected service offering. Values come from Healthify's Service Offerings taxonomy.

Each service offering belongs to one category for reporting purposes. For example, if the service offering is "Affordable food", which is within the "Food" category, the Service Offering Category Name will display "Food."

Additional Details Expected Waitlist End Date: The expected end date if the client is on a waitlist. Applicable if the action is “Client Enrolled in Service”. (Note: this field is optional.)

Additional Details Expected Service to Be Provided: The expected goods or services to be provided to the client. Applicable if the action is “Client Enrolled in Service”. (Note: this field is optional.)

Additional Details Expected Quantity: The expected quantity of goods to be provided. Applicable if the action is “Client Enrolled in Service”. (Note: this field is optional.)

Additional Details Expected Length of Time: The expected duration of services to be provided, split into quantity and time interval. Applicable if the action is “Client Enrolled in Service”. (Note: this field is optional.)

Additional Details Expected Service Frequency: Whether the expected goods or services will be one-time or ongoing. Applicable if the action is “Client Enrolled in Service”. (Note: this field is optional.)

Additional Details Service Provided: The goods or services provided to the client. Applicable if the action is “Service Provided”. (Note: this field is optional.)

Additional Details Provided Quantity: The quantity of goods provided. Applicable if the action is “Service Provided”. (Note: this field is optional.)

Additional Details Provided Length of Time: The duration of services provided, split into quantity and time interval. Applicable if the action is “Client Enrolled in Service”. (Note: this field is optional.)

Additional Details Service Completion Status: Whether or not the service is completed. “Incomplete” means the service will be repeated again. Applicable if the action is “Service Provided”. (Note: this field is optional.)

Additional Details Reason Service Was Not Provided: The reason for cancelation provided by the user. Applicable if the action is “Service Not Provided”. (Note: this field is optional.)

Client UUID: The unique identifier of the client who was referred.

Client Gender: The current gender of the client who was referred. (Note: Gender is not required for clients.)

Client Year of Birth: The current year of birth of the client who was referred, using HIPAA de-identification standards. Clients older than age 89 are grouped into a single group with the following format: "YYYY or Before".

Client 3-Digit ZIP Code: The current zip code of the client who was referred, using HIPAA de-identification standards. All client zip codes in the US are grouped together by the first three digits. For groupings with less than 20,000 people, the zip code is changed to "000." For groupings with 20,000 people or more, the three digits are shown. (Note: Zip code is not required for clients.)

Service Detail Sequence Index: The order this service detail was logged, when there are multiple service details for the same service offering within a referral.

Client Population Name List: The population tags of the client. Population tags are customizable per Company.

Client Intervention Name List: The intervention tags of the client. Intervention tags are customizable per Company.
