Need help figuring out what this report is and what it contains? Below you will find a brief report description as well as necessary data definitions.

The Healthify De-Identified Screenings Extract enables customers with screenings enabled to receive automatic exports of screening information, excluding identifiable patient information. Customers may receive this file on a selected time interval (typically monthly or weekly) delivered as a zipped CSV file via either email or SFTP upload.

Each row represents a single screening that was started within Healthify during the report's date range, and the file will be filtered by the Network or Company, as well as the defined date range (i.e. past month, week, day, etc.). The rows will be sorted in chronological order based on the Screening Started Time.

Data Definitions

Fields are listed in the order they appear in the De-Identified Screenings Extract.

Screening ID: The unique identifier of the screening. This is a numeric ID which represents a single screening administered to a client.

Screening UUID: The secure unique identifier of the screening. (Note: This is provided primarily for troubleshooting purposes, but serves the same purpose in data analysis as the Screening ID.)

Screening Opted Out: Whether or not the client accepts the privacy policy to continue the screening. “Yes” means the client opted out, and “No” means the client opted in. (Note: If the client opted out, the screening will be considered complete, so it will have a Screening Completed Date and a Screening Completed Time.) 

Screening Started Date: The date when the screening was started in the company or network's time zone. Formatted YYYY-MM-DD (although data analysis tools such as Excel may automatically convert dates to your system’s default date format).

Screening Started Time: The complete date and time when the screening was started in the company or network's time zone. Formatted in 24-hour time.

Screening Completed Date: The date when the screening was completed in the company or network's time zone. Formatted YYYY-MM-DD (although data analysis tools such as Excel may automatically convert dates to your system’s default date format). This field will be null if the screening has not been completed.

Screening Completed Time: The complete date and time when the screening was completed in the company or network's time zone. Formatted in 24-hour time. This field will be null if the screening has not been completed.

Screening User ID: The unique identifier of the user who started the screening within Healthify. (Note: Healthify supports multiple workflows which enable screenings to be administered without a logged-in user, including patient self-complete screenings and integrated screening workflows. This field will be null for screenings without a logged-in user.)

Screening User First Name: The current first name of the user who started the screening within Healthify. (Note: Healthify supports multiple workflows which enable screenings to be administered without a logged-in user, including patient self-complete screenings and integrated screening workflows. This field will be null for screenings without a logged-in user.)

Screening User Last Name: The current last name of the user who started the screening within Healthify. (Note: Healthify supports multiple workflows which enable screenings to be administered without a logged-in user, including patient self-complete screenings and integrated screening workflows. This field will be null for screenings without a logged-in user.)

Screening User Email: The current email address of the user who started the screening within Healthify. (Note: Healthify supports multiple workflows which enable screenings to be administered without a logged-in user, including patient self-complete screenings and integrated screening workflows. This field will be null for screenings without a logged-in user.)

Screening Team ID: The unique identifier of the team of the user who started the screening as of the screening start time. (Note: Healthify supports multiple workflows which enable screenings to be administered without a logged-in user, including patient self-complete screenings and integrated screening workflows. This field will be null for screenings without a logged-in user. Additionally, if the screening user has changed teams after starting the screening, this value will still represent the user's team was the screening was started, not the user's current team.)

Screening Team Name: The current name of the team of the user who started the screening as of the screening start time. (Note: Healthify supports multiple workflows which enable screenings to be administered without a logged-in user, including patient self-complete screenings and integrated screening workflows. This field will be null for screenings without a logged-in user. Additionally, if the team's name has changed at any point, this value will represent the most current name as of the time that the report was pulled.)

Screening Company ID: The unique identifier of the company associated to this screening. Always present, whether or not there was a screening user.

Screening Company Name: The current name of the company associated to this screening. Always present, whether or not there was a screening user.

Screening Template ID: The unique identifier of the screening template. A screening template represents the series of questions administered in a given screening and is unique to a company.

Screening Screening Template UUID: The secure unique identifier of a screening template. (Note: This is provided primarily for troubleshooting purposes, but serves the same purpose in data analysis as the Screening Template ID, to uniquely identify a screening template.)

Screening Template Name: The current name of the screening template. A screening template represents the series of questions administered in a given screening and is unique to a company.

Client UUID: The secure unique identifier of the client who was screened.

Client Gender: The current gender of the client who was screened. (Note: Gender is not required for clients.)

Client Year of Birth: The current year of birth of the client who was screened, using HIPAA de-identification standards. Clients older than age 89 are grouped into a single group with the following format: "YYYY or Before".

Client 3-Digit Zip Code: The current zip code of the client who was screened, using HIPAA de-identification standards. All client zip codes in US are grouped together by the first three digits. For groupings with less than 20,000 people, the zip code is changed to "000." For groupings with 20,000 people or more, the three digits are shown. (Note: Zip code is not required for clients.)

Client Population Name List: The population tags of the client who was screened. Population tags are customizable per Company.

Client Intervention Name List: The intervention tags of the client who was screened. Intervention tags are customizable per Company.

Matched Service Offering Name List: The service offering(s) that were matched to the client based on the responses to screening questions. Values come from Healthify's Service Offerings taxonomy. Provided as a comma-separated list sorted in alphabetical order.

Matched Service Offering Count: The count of the service offering(s) that were matched to the client based on the responses to screening questions.

Matched Service Offering Category Name List: The social determinants of health (SDoH) domain category(ies) of the matched service offerings for the screening. Values come from Healthify's Service Offerings taxonomy. Provided as a comma-separated list sorted in alphabetical order.

Each service offering belongs to one category for reporting purposes. Categories which contain multiple service offerings connected to the screening will only appear once. For example, if the matched service offerings list shows "Affordable food" and "Food pantries", both of which are within the "Food" category, the Service Offerings Categories List will display "Food."
