Search for resources, share their information, and contact organizations

The Community Access Platform allows you to search for social service programs within your community, share the information about these programs with yourself, and even contact some organizations directly! Click or scroll below to learn more about all the actions you can take in WellSky's Community Access Platform.

Search for resources

There are 3 fields to fill out when conducting a search:

  • Service offering: which specific service offering you are searching for. Ex: "Affordable Food, or "Child Care".

  • Distance: the mile radius from the address you enter to increase or decrease your search radius.

  • Location: the address, city, or zip code where you would like to locate resources in.

Selecting service offerings

In the Find field, begin typing the service offering you're searching for to view a dropdown menu. From here, you can choose one or multiple service offerings to refine your search.

Service offerings are organized by categories. Listed within each category are the related service offerings. Those with an arrow indicate more options within this category.

Choosing a mile radius

When clicking in the Distance field, you will see a dropdown menu with options from 1 to 100 miles. This is the radius from the location you provide in the next field, Location. 50 miles will always be the default distance.

Selecting a location

In the last Location field, type the address, city or zip code where you would like to see resources. Tip: The most accurate search will result from typing an exact address.

Any vs All

The Any feature will show results that match at least one of the service offerings you have added to the Find field. The more service offerings you provide, the broader your search.

The All feature will show results that match all of the service offerings you have provided (i.e. 12-step programs, Substance abuse, Job placement.) This will produce a more narrow and specific list of resources.

Click Search to generate a list of resources that match your search!

Understanding Your Search Results

Your search results will be represented by Resource Cards. Resource cards are condensed to show the most relevant information: a brief description, distance, and service offerings that match your search.

Resource Cards

To learn more about a resource, click on the resource’s name in blue on the Search results page. A resource page is divided into three sections:

  • Basic information

  • Resource overview

  • Next steps

The Basic Information contains the following information about a resource:

  • Program Name and Organization Name

  • Address

  • Last updated: when the resource site information was last verified by our Resource Network team

  • Phone number (if provided)

  • Email (if provided)

  • Website (if provided)

  • Hours (if provided)

  • Map

The Resource Overview contains a more detailed description as well as relevant additional information. You'll also find:

  • Service offerings: All programs and/or service types offered at a resource site

  • Eligibility requirements: All populations served at a resource site

  • Languages: All languages spoken at a resource site

The Next Steps section contains:

  • Next Steps: what an individual must do to access services

  • Capacity: whether a resource or agency can accept new members.

  • Wheelchair Accessibility: whether or not a resource is accessible by wheelchair.

  • Required Items: items that may be necessary for an individual to access services.

  • Price Range: the cost associated with services, if there is one.

Sharing Resource Information with Yourself

If you feel this resource would benefit you, and you meet any eligibility criteria for the resource, you can share the resource's information with yourself in two ways.

Texting a Resource

Select the blue Text Resource button to text the resource information to yourself. You will receive a message that will contain a link to the resource's card. Type in your cell phone number in the pop-up and select Send Text.

You can text STOP to the number if at any time you do not want to receive text messages from Healthify anymore.

Downloading a Resource

Select the More dropdown menu on the top right of the resource card, then Download to download a PDF version of the resource listing. You can also download the resource listing in a language of your choosing.

Contact Organizations

For some resource sites, you may see a blue Contact button. You can fill in details about yourself and actually get in contact with the organization. By filling in your personal details WellSky will send a referral to the organization on your behalf.

  1. Select the blue Contact button on the resource listing

  2. Fill in the fields

    1. Requested services: which services you are interested in

    2. First Name, Last Name, and Date of Birth

    3. Preferred Method of Contact - whichever method you choose, you will receive updates to the contact you choose

    4. Email Address and Phone Number

  3. Authorization: We ask for your authorization to send your information, including the request, to the organization and for you to receive updates back to your preferred method of contact.

  4. Select Contact when all the fields have been filled out and you are ready to send

  5. After sending the form, you will be asked if you want to also download the resource's information to a PDF document.

Organizations will respond to the form you send them, and you will receive updates to your preferred method of contact. Examples of these updates include:

  • The organization has accepted your referral

  • The organization has declined your referral (with a reason for cancellation)
