The Healthify Client Import Failures Extract enables customers that manage members in Healthify using a member flat file to receive automatic exports of records that failed during import. Customers may receive this file on an interval based on the frequency of member file import (typically monthly or weekly) delivered as a zipped CSV file via SFTP.

Each row represents a failed record during import and provides detail on the file name, and reason of error.

Fields are listed in the order they appear in the Client Import Failures Extract

Created Date: The date that the error occurred, when imported into Healthify.

Row Number: The row number of the record in the identified file that errored during import.

File Name: The name of the file that contains the record.

Client External ID: The current external identifier of the client that was imported. External ID is a generic field that Healthify customers may use to link client records in Healthify to corresponding records in a separate system, typically through integration or flat-file patient import. Often a medical record number (MRN) or member identifier.

Import Status: The current import status of the record row. A record being present on this file will read ‘Failed.’

Error Messages: The reason the record errored while being imported. Please see the list of possible errors below and their explanation.

List of Possible Errors

Phones number is an invalid number:

Phones number must have ten or eleven digits

Phones number can't be blank

Relates to:

Phones kind is invalid

Relates to:

  • TELEPHONE_NBR_TYPE

Identifiers normalized value can't be blank

This error typically occurs when the insurance field is missing a policy number.

  • MEDICARE_ID

  • MEDICAID_ID

Identifiers effective date must be after the client's date of birth

Relates to:

  • ADDITIONAL_IDENTIFIER_INFORMATION

  • MEDICARE_EFFECTIVE_DATE

  • MEDICAID_EFFECTIVE_DATE

Email Address must be a valid email

Relates to:

  • EMAIL_ADDRESS

Teams were not found

This error occurs when the external IDs presented are not yet mapped to team records in Healthify. This requires Healthify intervention.

  • TEAM_EXTERNAL_ID

Date format must be m/d/yyyy or mm/dd/yyyy

Relates to:

  • BIRTH_DATE

Patients date of birth can't be blank

Relates to:

  • BIRTH_DATE

Patients date of birth must be in the past

Patients date of birth must be after the year 1800

Relates to:

Terminated date must be m/d/yyyy or mm/dd/yyyy
